Responsibilities

  • Conduct initial assessments of clients' needs and develop personalized care plans

  • Coordinate and schedule home care services based on clients' requirements

  • Collaborate with clients, their families, and healthcare professionals to ensure the delivery of high-quality care

  • Monitor the implementation of care plans and provide ongoing support and guidance to caregivers

  • Assess caregiver performance and provide feedback and training as needed

  • Maintain accurate and up-to-date documentation of all client interactions and caregiver assignments

  • Stay informed about industry trends and best practices in home care services

Requirements

  • Bachelor's degree in a related field or equivalent work experience

  • Minimum of 2 years of experience in home care coordination or a similar role

  • Proven ability to assess clients' needs and develop customized care plans

  • Excellent organizational and time management skills

  • Strong interpersonal and communication skills

  • Ability to work independently and as part of a team

  • Proficient in using computer applications and software for documentation and scheduling
